Thank you for considering Advocates in Aging, Law Office of Ira S. Wiesner for your estate planning needs. The information you provide in this questionnaire will be used to help you organize your personal and financial information so we can properly assess your current situation and evaluate what documents are appropriate for you. 

 A “Confidential Planning Questionnaire” is available below for you to download and complete. We would appreciate your completing the Questionnaire in advance. Do the best you can. The more complete your answers, the better the attorney will be able to assist you. If you have any questions about the form, please call our office and someone will be happy to assist you. Of course, everything you disclose is confidential and will be held under the strictest confidence.

If you are unmarried, or if you are married, but wish to implement an estate plan separately from your spouse, simply fill out the SINGLE version of this form. If you are married and wish to implement a joint estate plan with your spouse, please fill out the MARRIED version of this form.

If at all possible, please return the Questionnaire to us at least two days in advance of your consultation. This will give the attorney an opportunity to review it and more can be accomplish during your meeting. If you are unable to return the Questionnaire in advance, please bring it with you to your appointment.
